Access Multi-Disciplinary Team Support
Frequently I refer on to other professionals involved in ensuring the health and independence of my clients. Often a team approach is needed. With your consent we can collaborate.
Referrals can be made to NHS services such as the Neighbourhood Teams, the third sector such as charities or sometimes a private option is taken these can include:

- Older Adult Mental Health Consultants – Dementia Diagnoses
