We witness regularly the stress increasing frailty can place on relatives and carers, who often need more support during times of crisis and ill health.

In this service, we conduct a detailed assessment of the patient’s current medications, evaluating their efficacy, potential side effects, and interactions. Weight Loss Reviews, 1 hr
Our doctors provide tailored Weight Loss Reviews for older adults. This holistic service involves an assessment of factors influencing weight, including metabolism changes, medical conditions, and lifestyle. Collaborating with patients, we create individualised plans, considering nutritional needs, physical activity levels, and potential causes of weight loss.
Ongoing support and monitoring are integral components of this service, ensuring that older individuals achieve and maintain a healthy weight in alignment with their unique health requirements.

“Dementia Diagnosis pathway” 2hrs
Full history, Memory assessment, physical examination, urinalysis, memory bloods, ECG and referral for CTH.
This is then collated and referred onto an older adult mental health consultant to review and discuss possible diagnoses.
Positive Ageing and Annual Health Screen (50 + age group)
Home Visit Healthcare offers a Positive Ageing Annual Health screen , focusing on promoting positive ageing in adults who are 50+.
This includes a thorough health assessment covering physical, mental, and social aspects of aging.
We provides a detailed report outlining evidenced based strategies to prevent frailty, enhance future mental, cognitive and physical wellbeing, and facilitate positive aging.
Emphasising preventive measures, this assessment empowers individuals to “live longer, living better” by proactively addressing their health needs, empowering through education regarding screening they require, vaccination programmes relevant to them to optimise their quality of life into retirement and older age.
